Clean and Smoke Free Dress*

My dress is clean (without multiple stains) and smoke free. I understand that if my dress is not in good condition (smoke free and without multiple stains), it may not be usable for the Angel Gowns Project.

Liability Agreement*

I understand that the dress(es) I supply to Real Imprints, LLC, will be cut and sewn into smaller Angel Gowns. I agree that I will not hold Real Imprints, LLC, The Angel Gowns Project, or any seamstress involved in the project liable for damage, loss, or other problems with the dress(es) I am providing.
